Nepotism is everywhere, even in the Naruto universe. In order to be the Hokage, you have to be a blood relative or a student of a previous Hokage.
Hashirama was the founder, of course he'd be the first.
Tobirama was his brother
Hiruzen is Tobirama's Student
Minato was the exception, but he was Jiraiya's student, who was a student of Hiruzen.
Tsunade was Hiruzen's student and Hashirama's granddaughter.
Kakashi was Minato's student
Naruto was Minato's son and Kakashi's student.
Notice how all of the Hokage are mentioned. Plus, two others that have strong claims to being the Hokage are Konohamaru Sarutobi and Sarada Uchiha.
Konohamaru was Naruto's student, as well as Hiruzen's grandson.
If Konohamaru became Hokage, then Sarada would be Konohamaru's student.
And her claim would be even Stronger if Madara didn't betray the village, because Hashirama originally intended for Madara to be the second Hokage. She'd have a blood tie.

I mean... Shikamaru could have been the Hokage. He certainly had the brains to be effective as one. But even then, his tutor was Asuma, who is Hiruzen’s son.
Madara could have become one. Hashirama was quite willing to give him the position, but he betrayed the village.
It’s possible that you didn’t have to have the whole student teacher thing going on, it’s just how it happened most of the time.
It probably would have been feasible for Rock Lee to become Hokage. But it wasn’t his goal in life. If he’s powerful enough to withstand a meteorite while only being able to use taijutsu, then he’s probably worthy of the title.
Although, Naruto has the advantage of being Asura’s reincarnation and a Ninetales sealed inside of him. That might have been his real advantage. But I don’t think many Jinchuuriki became Kage, save for the third Mizukage and Gaara.
