Can you check if this has been reported yet? There's a minor bug in the mirage tower where Taka's voice appears if you interact with the puzzle even if he isn't there, and if the puzzle is already completed. Funnily enough, this is the only room where Taka's voice appears, as the other rooms have no effect if you interact with the movable puzzle piece.
