Focus Sash/Band

Hey, (hope I can ask this in here)

I'm doing a third run on the game but I can't find this item. I know it exists because I had one in my other saves. Can anyone help me on this?

Focus Band is sold in the Department store.

Focus Sash is found on a rock in the "Beneath the great Stairs" area, partway down the leftmost path.

It is also found in the Wasteland, in a cave on the southeast area.
